Microsoft Forms

What is this Service? 

Microsoft Forms is an easy‑to‑use tool in Office 365 that lets you build surveys, polls, or quizzes in minutes. Drag‑and‑drop questions, add pictures, videos, or your logo, and share a link, QR code, or embed code. Responses appear instantly and can be saved to OneDrive, Teams, or SharePoint (and exported to Excel) for further analysis.

Who is Eligible to use it?

Students, faculty, staff, alumni, and authorized guests with a Temple Microsoft 365 account can create forms. Anyone you invite - inside or outside Temple - can respond, unless the form uses the file‑upload question (see Limitations).

How do I use it?

  1. Go to forms.office.com and sign in with your Temple email and password.
  2. Click New Form or New Quiz, then add questions such as multiple choice, text, or rating.
  3. Customize with images, video, or your department logo.
  4. Select Collect responses and choose whether Anyone or Only people in my organization can respond. Copy the link, QR code, or embed HTML and share it.
  5. Watch live results in the Responses tab or click Open in Excel to download data.

Cost and Limitations

  • Microsoft Forms is included in the University’s Microsoft 365 license—there is no additional charge to create or use forms.
  • If your form includes a File Upload question, only logged‑in Temple users can respond (external users cannot upload files).
  • Custom workflows or Power Automate integrations are self‑service and not supported by ITS.
  • Storage limits follow your OneDrive, Teams, or SharePoint quota.
  • Service availability depends on Microsoft’s cloud uptime; major outages will be posted on System Status.
