Offices, departments, or individuals interested in administering a survey to a large group or sample from the Temple community, must complete this survey administration application. The application will gather information on the purpose, design, and time frame of the proposed survey.
Please note, before completing this application, review the Survey Administration Process. Additionally, all research projects involving human subjects must be reviewed and approved by the Institutional Review Board (IRB). The survey sponsor (i.e., the individual who submits this application) must obtain IRB approval or confirmation from IRB that the survey does not require IRB review.
Once submitted, the application will be reviewed by the Office of Institutional Research and Assessment to ensure all materials are enclosed. Applications will be reviewed as quickly as possible, but the TUSCC often requires additional information, which may add time that should be included in the survey administration schedule.
